- ➤ An investigation is underway after Larry Lewis, a 62-year-old man from Haviland, Ohio, died while sedated during a procedure on March 13 at a local dentistry facility—emergency personnel attempted life-saving measures until he was transported and he was pronounced dead on March 26—with an autopsy completed that did not release the cause or manner of death.
